1. Assesses patient's condition, learning needs and family dynamics based on systematic data collection having mastered assessment skills and tools used in the respective practice area. 2. Develops a comprehensive plan of care and discharge plan based on the nursing assessment and integration of the medical plan utilizing patients, family and other health care team members. 3. Implements patient care and prioritizes and collaborates with health team members as necessary. 4. Coordinates and delegates the delivery of care provided by selected members of the health care team. 5. Evaluates outcomes of the patient care planning and teaching, collaborating and coordinating with other health care team members to revise the plan according to the changing needs of the patient. 6. Communicates effectively and appropriately to patient, family and other health care providers. 7. Participates in the orientation and education of personnel and students. 8. Maintains clinical competencies (to include population specific competencies) in two specialties, in one critical care area, or in one specialty with increased scheduling commitment. Assumes responsibility for acquiring knowledge and experiences and integrates, as appropriate, into patient care delivery based upon specific patient needs. 9. Documents delivery of health care and nursing process in accordance with specific unit standards and hospital nursing policy.
Minimum Qualifications
Bachelor's or Master's in Nursing is preferred. Two (2) years of Registered Nurse (RN) experience is required. Temporary or permanent license to practice professional nursing (RN) in the State of Texas is required.
