Job Description:

In this position you will be responsible for coordinating care of patients in collaboration with nurses in the field. You will assign cases and provide support to field staff. You will also speak with contract vendors of HHA and equipment as needed.

Job Requirements:

RN, BSN preferred. Must have a minimum of 2 years home care experience in CHHA and or Longterm. PC skills a must. Good oral and written communication skills. Good telephone skills.
