Job Description:
Provides high-tech home care visits and assumes on-going primary responsibility for an assigned caseload of patients within a designated geographical area. Responsible for submitting documentation of patient visits in a timely and accurate manner. Communicates pertinent findings to case management coordinator. Required to work one weekend per month and designated holidays.

Job Requirements:
Associates/Diploma from an accredited school of Nursing; Bachelors Degree preferred. Minimum of one-year acute, Medical-Surgical nursing experience. CHHA / LTHHCP experience preferred. Documented experience working with vascular access devices and in the administration of intravenous therapies. Word and Excel knowledge preferred. Licensed with current Registration to practice as Registered Professional Nurse in NYS. Ability to insert peripheral vascular access devices, and perform venipuncture to obtain blood specimens. Excellent customer service skills. Ability to work within an integrated health care team. Ability to demonstrate effective critical thinking skills. Ability to problem solve independently. Ability to communicate in English both verbally and in writing. Proficiency in interviewing and physical assessment skills.
