Job Description:

Provides home care visits and assumes on-going primary responsibility for an assigned caseload of patients within a designated area. Responsible for submitting documentation of patient visits in a timely and accurate manner. Communicates pertinent findings to case management coordinator. Required to work one weekend per month and designated holidays.

Job Requirements:

Associates/Diploma from an accredited school of Nursing; Bachelors Degree preferred. Minimum of one-year home care experience required. CHHA / LTHHCP experience preferred. Word and Excel knowledge preferred. Licensed with current Registration to practice as Registered Professional Nurse in NYS. Excellent customer service skills. Ability to work within an integrated health care team. Demonstrates effective critical thinking and problem solving skills independently. Ability to communicate in English both verbally and in writing. Proficiency in interviewing and physical assessment skills.
