Position Description
Provides diabetes education activities for inpatients, outpatients, and individuals and groups of adult patients with diabetes. Coordinates the diabetes education plan with other team members, and acts as liaison to the primary nurse and physician. Serves as resource to other health professionals in the specified area of expertise for the purposes of staff development, patient care delivery, and Quality Improvement. Must be willing to work flexible hours, including weekends, evenings, and weekend call. Must complete a minimum of twenty four (24) hours of continuing education in diabetes and education principles during initiation on job, and must complete six (6) hours of diabetes education.
Position Requirements
Required:
1. Graduate of an accredited nursing school with one (1) year experience in Diabetes Patient Care, or patient education.
2. Good written and oral communication skills.
3. Ability to work effectively with a multidisciplinary team.
4. Demonstrated knowledge of adult education principles.
5. CDE within two (2) years of hire.

Preferred:
Certified Diabetes Educator

License/Certification Requirements: Yes

Work Hours: 8a-4:30p

Weekend Requirements: No
