As a Community Crisis Center RN, you will need to know how to:
Provide nursing care to patients and be responsible for patient care coordination, telephone triage, team leading and supporting staff development in the clinic environment Use and document levels of care criteria for all patients throughout care continuum and develop and direct continuum of care map in collaboration with interdisciplinary team (IDT) Identify obstacles to efficiency and goal outcomes and intervene to overcome or eliminate these when possible Assess patient needs, coordinate patient flow, administer care and revise plan of care accordingly Demonstrate caring and involved behaviors to patient/family members and play an advocacy role to ensure positive outcomes Perform phlebotomy tasks, collect and log specimens, assist in the coordination of lab result reporting processes and perform quality control checks Prepare and instruct patients for radiology procedure and perform site specific chest and extremity procedures according to policies and procedures Your experience.
