Nurse Practitioner- Skilled Nursing Facility

Position Summary

Under the general supervision of a collaborating physician, coordinate the comprehensive health care needs of acute-care residents in a Skilled Nursing Facility. Provide direct care through assessment, diagnosis, and treatment of acute minor and chronic conditions within the scope of the Advanced Practice Nurse. Support and provide a true continuum of care to skilled nursing facility residents, working collaboratively to improve quality outcomes and reduce avoidable readmissions.

Job Summary

DUTIES AND RESPONSIBILITIES: All duties will be carried out according to facility policy and in accordance with applicable regulations regarding the practice of NPs in the Commonwealth of Pennsylvania and all Long-Term care regulatory bodies. Manage both medical and behavioral chronic, episodic and acute conditions effectively in collaboration with attending physician; consult with specialty providers as needed and ensure information is shared with attending physician. Manage transitions in care at admission and discharge Perform comprehensive assessments and document findings in a concise/comprehensive manner that is compliant with documentation requirements and Center for Medicare and Medicaid Services (CMS) regulations Assure that all encounters are accurately documented including problem, assessment, plan, diagnosis and ICD10 code(s). Work collaboratively with the Supervisory Physician, SNF Medical Director and SNF Director of Nursing to review Quality Measures and Best Practices. Manage plan of care by engaging the facility staff, families/responsible parties, primary and specialty care physicians Evaluate the effectiveness, necessity and efficiency of the plan of care, making revisions as needed Plan and initiate health maintenance and disease prevention for patients, including education, instruction and counseling. Provide education to staff as appropriate to further advancement of care and clinical outcomes. Participate in professional meetings, committees, and seminars Participate in on-call coverage and rounding on weekends and holidays



REQUIRED KNOWLEDGE, SKILLS, ABILITIES, AND EXPERIENCE Proficient computer skills including the ability to document medical information with written and electronic medical records (including baseline documentation in Epic) Understanding of Geriatrics and chronic disease management Ability to develop and maintain positive relationships with RHS employees, families and residents. Adaptability to change Understanding of advanced illness and end of life discussions Certified nurse practitioner through ANCC/AANP for adult, acute care, family and/or geriatrics required Current unrestricted license in Commonwealth of Pennsylvania
