Nurse Navigator RN Temple Cardiovascular Med Full Time (Relo - TX - Sign On Bonus)

The Nurse Navigator Registered Nurse (RN) facilitates the patients continuity of care with the healthcare team in an efficient and timely manner. The Nurse Navigator RN acts in an intake model, assesses the patient needs, assist with planning, organizing, implementing and evaluating the patient care needs with the healthcare team. The Nurse Navigator functions autonomously and in collaboration with other members of the healthcare team as a patient advocate, educator and coordinator. The Nurse Navigator RN coordinates with physicians office the testing to be performed in order to make a definitive diagnosis. The Nurse Navigator RN educate patients and family members on diagnosis, treatment options, available services and provide direction and support to the patient/family throughout the continuum of care. The Nurse Navigator RN participates in continuous quality improvement projects in order to better serve the patient, family and healthcare system to improve the quality of service provided.

The ideal candidate has strong verbal and written communication skills.
