Essen Health Nursing Home Division is an extensive network of clinicians who provide high quality medical care to nursing home residents located at more than 42 skilled nursing facilities across all New York City boroughs, as well as Westchester and Duchess Counties.   • Maintain accurate records on patients treated in EHR
  • Conduct comprehensive adult/geriatric physical examinations recognize conditions, characteristics and behaviors that place this population at risk
  • Diagnose and treat acute illness and manage chronic diseases
  • Liaison between the individual, family, and health care providers: physicians, social workers, dietitians, OT/PT therapists as necessary regarding health status of patients

  • Must have current New York State license as a Nurse Practitioner (ANP, FNP, GNP)
  • Board Certification
  • Long-term care experience or geriatrics experience preferred
  • Experience using electronic health records
