JASA Long Island Center for Dignity and Support

The Long Island Center for Dignity and Support seeks to provide care and services to Long Island's older adults and those struggling with a serious, life-limiting and advanced illness and loss through a holistic approach addressing the emotional, physical and spiritual needs of the patients/clients. The nurse will integrate expert health case management services for Long Island Network for Dignity and Support clients in collaboration with the program's other staff.

Primary Responsibilities:

· Conduct in-home nursing assessments of clients with health concerns to identify risks and coordinate their health and long-term care with a focus on such issues as: polypharmacology, nutrition, vision and cognitive decline, depression, home safety, symptoms, treatment options, pain and symptom management, and medical equipment

· Promote maximum level of wellbeing among clients, assisting them to develop a management plan to take an active role in their healthcare

· Consult with staff regarding medical issues and goals of care by meeting on a regular basis and providing joint visits with program clients/patients

· Discuss preparation of health care proxies, MOLSTs, and other advanced directives

· Consult with current community and hospital care team and advocate for additional or alternative treatment as needed

· Provide treatment as needed and appropriate (as long prescribed by physician or NP) such as wound care and pain and symptom management

· Provide community education programs to both general community and professionals

· Participate in evaluation activities and provide qualitative and quantitative information as needed for the project


· Current NYS Registered Nurse License; ACLS/BCLS Certification, Master's degree in nursing preferred

· 3-5 years of experience in Geriatrics and/or Palliative Care preferred

· Proven experience in training staff, other professionals and general community

· Desire and ability to work collaboratively and thoughtfully as part of a committed, integrated team, and build strong relationships

· Excellent interpersonal skills, including the ability to communicate effectively and concisely (both in writing and orally)

· Strong attention to detail with the ability to solve problems, prioritize work, and work independently

· Basic computer skills; familiarity with EMR preferred

· Ability to drive to visit clients and other partners as well as take mass transportation

· An understanding and appreciation of the roles of race, religion, ethnicity, sexual preference, and individual values have as they relate to serving clients
