We are a 292-bed acute care hospital
, located in the upbeat, North Chesterfield area, a short drive from Chesterfield Town Center, offering an abundance of restaurants and shops. The Rehab Center

Newly renovated, our unit includes 38 spacious private patient rooms (6 that are double)
, two large patient therapy gyms located on site, laundry, accessible kitchen and dining room for patients. We provide therapy for all body parts including hands, provide stimulating activities for the mind, and we received the Joint Commission Basic Stroke Certification in 2016.

As a Clinical Rehabilitation Specialist
you will act as a liaison between internal and external referral sources and internal operations of the Rehab Unit.  You will be responsible for establishing relationships with physicians, payers and other referral sources.  In addition, you will ensure appropriate patient placement through clinical assessments and screening in conjunction with the Rehab Medical Director/Physiatrist. 
 

  • Completes history and physical prescreening assessment of patients to determine qualification for admission to inpatient rehab.

  • Collaborates with medical dir4ector, staff physiatrists and admissions committee regularly.

  • Acts as a liaison and resource between the Rehab Unit, referring physicians, social workers, case manager, patients, and families.

  • Communicates with third party payers for pre-certification and recertification of the rehab stay.  Verifies insurance coverage for the rehab stay.

  • Assists marketing with broadened rehab referral base.

  • Establishes and maintains strong working relationship with HCA case managers in all area facilities.

  • Provides education to referral sources regarding rehabilitation regulatory requirements.

  • Daily travel is required to each referring facility.
