Job Description

Reporting to the Director of Home Care is responsible for the coordination of Home Care patient care services. Coordinates activities of nursing staff in order to ensure that patient needs are met. Provides leadership to build a partnership between the staff, patients and family, medical staff, the community and the organization.

>Will oversee the assignment of appropriate staff to all cases.

>Assures all staff members are appropriately oriented for the cases assigned.

>Assures compliance with the state, federal, and local laws.

>Conducts counseling sessions, terminations, etc., field staff.

>Coordinates and assists the interdisciplinary team in the development and implementation of patient policies.

>Assures accuracy of visit delivery in conjunction with order frequencies.

>Reviews patient medical files on a regular basis and addresses any discrepancies noted.

>Assists QA Manager in identifying educational activities for staff including in-services based on employee needs and promotes staff development.

>Supervises day to day patient care activities and is available to field staff for assistance with problems or concerns.

>Supervises Schedulers/ Service Coordinators and perform scheduling activities in their absence.

> Maintains professional growth and development through seminars,workshops,and professional affiliations to keep abreast of the latest trends in the field of expertise.

>Able to work flexible schedules , rotate on call and perform patient visits when agency need dictates.

Requires Bachelors degree in Nursing and minimum 3 years Home Care clinical experience.
