What You Will Do:

University of Maryland Medical Center's Community Psychiatry Division is currently seeking a Health Home Nurse Care Manager
  • Under the direction of the Nurse Practitioner and Clinic Manager, and utilizing proficient somatic skills, the Health Home Nurse Care Manager oversees and performs the day-to-day functions of the Behavioral Health Home in Community Psychiatry.
  • This position is accountable for designing and maintaining systems to ensure the continuity of care between the community psychiatry ambulatory programs and somatic care providers in the community, as well as developing processes to assist consumers in the development and maintenance of healthy lifestyles and illness management skills.

Health Home Manager Responsibilities
  • Leads practice transformation based on Health Home principles and state/Joint Commission requirements.
  • With agency leadership, acts as a “champion” for the Behavioral Health Home philosophy and provides education to staff.
  • Designs and develops prevention and wellness initiatives.
  • Collaborates/consults with specialty practices to identify data needs related to care coordination and implements performance/outcome measures. Utilizes performance/outcome data to drive performance improvement.

Nurse Care Manager Responsibilities
  • Acts as a comprehensive care coordinator for participants assigned to care, assessing their needs accordingly and facilitating communication amongst other clinicians providing care.
  • Perform comprehensive assessments of physical, emotional, psychosocial and environmental needs of participants.
  • Facilitates health education groups.
    • Initiates client education based on identified learning needs of the client and/or those providing care and documents appropriately.

What You Need to Be Successful:
  • BSN Required. 3 years’ experience in clinical nursing.
  • Minimum of 2 years’ supervisory, program coordination, or leadership experience.
  • Licensure as a Registered Nurse (RN) in the state of Maryland, or eligible to practice due to Compact state agreements outlined through the MD Board of Nursing, is required.
  • Demonstrated success in a leadership role exhibiting teaching, coaching, and development skills. Demonstrated effectiveness in a consultative role, and experience leading complex change.
  • Highly effective interpersonal, verbal, and written communication and presentation skills. The successful candidate will possess tact, diplomacy, and adaptability.
  • Ability to independently develop and manage programs and projects. Ability to plan, organize and follow through on assignments and projects.
  • Technically proficient in Microsoft Office programs and database use. Ability to learn and use an electronic medical record and health information exchange to enter clinical information and run reports.
