Clinical Care Coordinator

We have an exciting opportunity to join our team as a Clinical Care Coordinator (CCC). The CCC provides clinical leadership and coordination for patient care consistent with the philosophy and goals of the institution, the Department of Nursing, and Care Management Department. Develops/manages the patient care transition process: coordinating, facilitating and assisting patients throughout the episode of care. Serves as clinical resource with expertise in musculoskeletal patient care management and serves as liaison regarding services for this patient population. Oversees the process for clinical pathway development, staff training, and data collection and reporting. Serves as a positive role model as a nursing leader. Evaluates and supervises care coordinated by staff in the Care Management Department. Demonstrates ability to develop, guide, motivate, nurture and coach others. Actively participates in quality initiatives.

To qualify you must have Bachelor’s Degree in Nursing, Masters of Science in Nursing or relevant health related field preferred. Current Registered Professional Nurse in the State of New York and membership in pertinent professional organizations; certification as a Case Manager or Nursing Administration is a plus; In addition, a minimum of three to five years clinical experience, acute medical-surgical preferred, or in the care of the population to be care managed with two or more years in utilization management, discharge planning/case management preferred. Prior Case Management and leadership experience are a plus. Must possess excellent interpersonal communication skills, customer service orientation and creative problem solving skills are required. Qualified candidates must be able to effectively communicate with all levels of the organization.
