What You Will Do:

General Summary

Under general supervision of the Manager, the Patient Navigator, acts both proactively as well as in response to specific care needs, to assist the client to access services throughout the continuum as well as play a role in the elimination of individual and system barriers to accessing services. This position requires results-oriented collaboration with the appropriate leadership and staff at UMROI , UMMS and community based programs as required. This position will collect data and work collaboratively with leadership to improve the following : long-term health of the designated client population and their clinical outcomes, awareness and appropriate utilization of UMROI, UMMS and community services, client and caregiver satisfaction, and reduce the cost of health care for the designated client population.

Must demonstrate the knowledge and skills necessary to provide care appropriate to the age, culture, ethnicity, and specific individual diagnostic needs of the patients served in his/her department. He/she must be able to assess and interpret data about the patient’s status in order to identify each patient’s specific needs and provide the care needed by the patient group (pediatric, adolescents and/or geriatric patient group, culture and diagnostic specific).

Consistently expresses and demonstrates compassion and courtesy for patients. Extends courtesy and support to patient's families/visitors.

Principal Responsibilities and Tasks

The following statements are intended to describe the general nature and level of work being performed by the person assigned to this classification. They are not to be construed as an exhaustive list of all job duties performed by personnel so classified.

1. Respect for individuals, Customer Service, Teamwork and Collaboration:
  • Identify and collaborate with UM ROI clinical program teams, UMMS partners such as Case Managers, Shock Trauma Transitional Care Navigator, outpatient programs, and with community partners to facilitate a continuum of care to meet customer needs. Foster a well-coordinated care experience for the client, family and care providers.
  • Interpersonal Domain-Customer Service: Model excellence in customer service behaviors in daily practice. with clients and internal and external customers. Identify and act on opportunities to respond to customer requirements. Take action to prevent potential customer complaints. Foster a well-coordinated care experience for the client, family, care providers. Incorporate excellent customer service skills into daily practice

2. Leadership, Clinical and/or Technical Skills:
  • Assist patients and caregivers in Navigating through appropriate services in the continuum. This includes identifying and mitigating barriers, providing education/explanation regarding the need for services, inquiring about barriers to accessing services, being supportive of psychosocial concerns/needs, and coaching patients to formulate questions that should be asked during appointments.
  • Facilitate participation in services by identifying clients and ensure that they attend services that have been prescribed/recommended, coordinate communication with direct care providers to ensure the team is aware of the plan, integrate information ensuring that patient data is documented and shared as needed, and seek collaboration, enlisting other providers in addressing the patient's concerns.
  • Maintain systems that include identifying potential clients, internal and external to UMROI, who would benefit from the coordinated services and resources provided by the Navigator, building networks and a consistent referral practice that would be followed when onboarding a new client that include explanation of the navigator role and referral criteria, and review cases to mitigate process barriers.
  • Documents activities such as recording steps taken on behalf of the patient in the medical record, or separate navigation file and processing other necessary information relevant to the role.
  • Participates in business development regarding specific diagnostic programs.
  • Serves as a resource for community educational events such as lectures, staff education, and community outreach.
  • Adheres to regulatory standards and participates in regulatory survey preparation.
  • Performs related duties as assigned

3. Quality and Patient Safety:
  • Participates in program quality initiatives
  • Implement and maintain appropriate databases that may be utilized for programmic review.

4. Cost Effectiveness:
  • Explore strategies and work collaboratively with appropriate clinical teams to ensure timely access to services and appropriate resources, and clients' and clinical programs' utilization of resources.
  • Complete and submit all required written documents accurately and timely.
  • Contribute to the departmental budget process.

5. Education and Research::
  • Maintain current knowledge/expertise in the area of specialty. Attend and participate in department meetings, in-service training and educational programs for continued professional growth and development.
  • Implement and maintain appropriate databases that may be utilized for programmic review as well as research endeavors.

What You Need to Be Successful:
  • Current licensure as a Registered Nurse, Social Worker (LCSW), Physical Therapist, Occupational Therapist, Recreational Therapist or Speech Language Pathologist in the state of Maryland is required.
  • Current CPR certification
  • Masters degree in Social Work is required for Social Workers.
  • Bachelors of Science in Psychology
  • Bachelors degree in Nursing is required for Nurses.(CRRN is preferred)
  • Bachelor, Masters or Doctorate degree is required in Occupational, Physical or Recreational Therapy or Speech Language Pathology
  • A minimum of three years experience in a hospital/managed care setting is required.
  • Experience in utilization management, case management, and expertise with client diagnosis preferred.
