CHPG Women's Health Porter (formerly Aracea Women's Health) is looking for a full time Nurse Practitioner to join their team.

Assumes responsibility and accountability for rapid assessment, nursing diagnosis and works with MD partners to assure safe patient diagnostic testing and optimal plan of care. Provides comprehensive health assessment, diagnosis and treatment. Maintains prescriptive authority. Clinical decisions are formulated in consultation and collaboration with attending physicians. The Nurse Practitioner is additionally involved in education, consultation, performance improvement and research. Utilizes the nursing process to promote/restore optimal health and well-being of the patient. Assumes personal accountability for organizational mission and core values.

Graduate of Accredited School Of Nursing.

Masters degree in Acute Care Nurse Practitioner or Adult Nurse Practitioner

work experience requirements

|

|

2 years’ nurse practitioner experience preferred.

Ability to demonstrate complex clinical skills within the framework of the nurse practitioner education and guidelines.

Advanced physical assessment and critical thinking skills required.

Ability to listen, interact and communicate - both verbally and in writing.

Nursing license in the state of Colorado

Current Registered member of the Colorado Advanced Practice Registry.

Current BLS certification required from American Heart Association

Position duties (essential functions, INCLUDE % OF TIME)

DOCUMENTATION - 15%

  • Thorough and confidential maintenance of patient charts and records.

  • Documents ongoing status, interventions, patient response and outcomes in accurate, timely manner.

  • Assesses patient/family learning style and appropriately teach and document understanding

PLAN OF CARE – 30%

  • Engages patient/family to foster commitment to comply with plan of care.

  • Collaborates with interdisciplinary team to develop and implement holistic, individualized plan of care. Performs History & Physical Examinations. Order, obtain & interpret laboratory tests.

  • Establish medical diagnosis(is) and determine a plan of care.

  • Delegates to non-RN personnel in accordance with State Board of Nursing and hospital policy.

  • Assess patients’ physical, psychosocial, cultural and spiritual needs through observation, interview, reviews of records and clinical data.

  • Evaluates patient’s response and intervene in a timely manner to ensure optimal patient outcome. Dictates H&Ps and discharge summaries

SAFETY – 25%

  • Implements patient plan of care and observes outcomes in accordance with nursing standards and physician direction.

  • Ensures safety of each patient in least restrictive manner.

PATIENT ADVOCATE – 15%

  • Serves as patient advocate. Anticipates needs of provider, staff patient and family. Provide consultation and education to patients and families.

  • Collaborate with all members of the healthcare team in planning, implementing, monitoring and evaluating care delivery for the neurologic/neurosurgical patient in a variety of settings.

  • Effectively uses time, personnel, equipment and supplies to provide high quality, cost effective delivery of patient care.

GROWTH – 15%

  • Participates in community-based activities.

  • Organize process improvement and multidisciplinary clinical outcome activities utilizing advanced nursing expertise.

  • Serve as a resource to nursing and clinical staff. Generate educational opportunities for staff.
