Our Patient Care Navigator
 assist patients and their families in identifying and accessing appropriate institutional and community resources. They implement appropriate care intervention and follow up to ensure patients receive timely care, preventing delays and access to appropriate services.They make initial and ongoing assessments of patient needs (clinical, emotional and social) and makes appropriate recommendations or referrals for care. Our navigators, also facilitate the timely completion and confidential reporting of diagnostic testing results to patients. 


In addition they work alongside and collaborating with the team of patient, family, and healthcare providers in providing patient care in a safe, healing, humane, and caring environment while providing learning opportunities for patients/family members and team members alike. The navigator is critical in providing health information and documenting patient interactions in a manner consistent with hospital standards.In addition, they create reports from patient data such as volumes and outcomes.


This is a PRN position.
