Position Description
The Diabetes Educator will be responsible for providing diabetes education activities for inpatients, outpatients, and individuals and groups of adult patients with diabetes. Coordinates the diabetes education plan with other team members, and acts as liaison to the primary nurse and physician.
Position Requirements
Graduate of an accredited nursing school with one (1) year experience in Diabetes Patient Care, or patient education. Good written and oral communication skills. Ability to work effectively with a multidisciplinary team. Demonstrated knowledge of adult education principles. CDE within one (1) year of hire.

License/Certification Requirements: Yes

Work Hours: 8a-4:30p

Weekend Requirements: No

On-Call Requirements: No

Diabetes & Nutrition Services at Northside Hospital offers the communities who count on us for comprehensive diabetes, nutrition, and weight management services. The focus of our efforts? To help
people with diabetes learn more about their disease while developing the skills needed to make healthy lifestyle changes. It’s these life-long changes that work to minimize the risk of complications from
diabetes and while empowering patients and educating families.

Patients are supported by our multidisciplinary team approach to nutrition which includes interactions
with registered dietitians, clinical exercise physiologists, and other health specialists. Our program is accredited by the American Diabetes Association. For professionals who want to make a difference
