  The Case Management Support Director is responsible for the continuous monitoring of performance metrics and onsite assistance and in depth evaluation of the Case Management functions in the identified LifeCare facilities. This position is responsible for providing “extended” onsite assistance to facilities that have been determined to need additional assistance. This position provides direction for the administration of the Case Management function in the hospitals. The Case Management Support Director is actively involved in department activities, transdisciplinary team activities, and LifeCare activities to improve the care coordination process and strengthen identified weaknesses in the facility.

Patient Population: Has contact with patients in a non-clinical setting; understands and demonstrates appropriate behavior when interacting with patients in the adult (18-65 years of age) and geriatric (65 and older) populations.

Essential Functions:
  • Monitors performance of hospital’s case management departments to identify shortcomings and improve facility compliance related to care coordination functions, resource utilization, and outcomes.
  • Provided extended onsite assistance, to identified facilities, directing processes to strengthen the care coordination and documentation improvement activities, ensuring compliance with policy and procedures, government regulations and established protocols.
  • Completes regular onsite training and orientation for new case managers.
  • Develops and enhances relationships with the Physician Advisor to improve outcomes for identified cases and support documentation initiatives.
  • Provide coverage and case management department support for DCM vacancies.
  • Promotes hospital Case Management collaboration and communication with Business Development and Clinical Services departments.
  • The knowledge and skills necessary to perform position requirements are demonstrated through the successful completion of competencies established for the position, to include population served, and other special needs of patients and customers, served by the department.
  • Adheres to LifeCare policies, procedures, all safety plans and all standards imposed by regulatory organizations.
  • Regular attendance and timeliness is required.


Supervision: Has indirect oversight of LifeCare Hospitals' Case Management staff.

Experience: Minimum of ten (10) years of clinical experience, with 8 years of case management and utilization management experience. At least five years management experience in the case management environment. Prefer experience in a multi-facility healthcare system. Experience should include QIO, WPS, RAC or other types of internal and external audits; knowledge of utilization management principles, Medicare benefits and insurance processes, and contracts related to the long term acute care hospital setting.

Education: Degree in nursing required. MSN preferred.

Licensure/Certification: Currently licensed as a Registered Nurse. Certified Case Manager preferred. Must maintain current licensure while employed with LifeCare Hospitals.
