Clinical Nurse Specialist-Primary Care Mental Health Integration (PCHMI)
The Clinical Nurse Specialist (CNS), Primary Care Mental Health Integration (PCMHI) is located in the Mental Health (MH) Service. The Clinical Nurse Specialist provides mental health services for any eligible Veteran requesting service to include, but are not limited to:
?The CNS PCMHI is responsible for supporting behavioral health care provided by primary care teams (Patient Aligned Care Teams (PACT)) that include PCMHI Collocated Collaborative Care Providers and Care Managers.
?The PCMHI CNS will be collocated with Primary Care (PC) and provide both curb-side and electronic consultation to Primary Care Provider's (PCP) regarding the prescribing of psychotropic medication within Primary Care. The PC-MHI CNS will also maintain a patient panel consistent with the PC-MHI treatment milieu and goals.
?The PCMHI CNS will be responsible for clinical oversight of PCMHI Care Management, overseeing patient panels and clinical outcomes using an electronic registry capable of tracking clinical processes and patient outcomes (Behavioral Health Lab).
?The PCMHI CNS will suggest treatment plan changes, including medication recommendations and supporting additional behavioral health treatment for patients who are not improving as expected through referral management. Additional responsibilities include the following: provide same-day access for immediate non-emergent psychiatric needs within Primary Care, as well as supporting psychiatric walk-in needs to the Urgent Care, on a limited basis (i.e., patient is active with psychiatry within Specialty MH, however, would be more expediently/appropriately cared for by the integrated CNS psychiatric provider).

- Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN), previously known as the National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
- Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
- English Language Proficiency. RNs appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).
- Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
- Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
- Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
- Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
- Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree.

Note: Must have Basic Life Support (BLS) certification prior to assuming clinical duties. The BLS requirement is satisfied by successfully completing an American Heart Association (AHA) BLS Healthcare Provider Course or Military Training Network (MTN). BLS courses are normally offered within 1-5 days of entry on duty. No online certifications will be accepted.
