||We have an immediate opportunity for a Home Care RN Case Manager.
General Summary: Coordinate client care with the agency team including physician, client/family and referring agency; and administer skilled nursing care for clients of all ages in their place of residence and assumes the responsibility for coordination of care.
Patient Population: Has contact with patients of all ages in a clinical setting; understands and demonstrates appropriate behavior when interacting with patients in all populations.
Case manages and provides clinical directions to the agency team, physician and family. Implement, develop, and document the plan of care. Provide care utilizing infection control measures that protect both the staff and the client (OSHA). Assess home health clients to identify the physical, psycho social, and enviromental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations. Assure continuity of quality client care delivered with appropriate documentation. Maintain a patient-centered service philosophy. Assume responsibility for continuity, quality, and safety of services delivered in compliance with Federal and State regulations and company policies. Review assigned cases to ensure compliance with requirements of third party payer. Review clinical records to ensure compliance with state and federal regulations. Apply concepts of infection control and universal precaution in coordination/performing client cares activities to protect both client and staff (OSHA). Completes appropriate documentation in a timely manner to assure compliance with company policy. Performs on-call responsibilities and provides on-call services to clients and their families as assigned. Demonstrates commitment and professional growth by participating in in-service programs and maintaining/improving competency. Meet mandatory continuing education requirements of the agency/licensing board. Promote the company philosophy and administrative policies.This description is a general statement of required essential functions performed on a regular and continuous basis. Supervision: Home Health Aides and LPN/LVNs
Experience: Minimum on two (2) years experience as a registered nurse. Home Health experience preferred.
Education: Completion of an accredited Registered Nursing Program. BSN preferred.
Current license that is in good standing to practice as an RN in any/all applicable states. Current CPR certificate. Current driver’s license in good standing.On-going employment as a Clinical Coordinator requires maintenance of a valid RN license in the state which the nurse practices in and a valid CPR certificate at all times. It is the responsibility of the employee to renew these both before they expire in order to continue employment.
Prolonged sitting required, with ability to operate office equipment. The ability to lift/transfer up to 50 lbs. Ability to meet deadlines and patient needs, make quick decisions and allocate resources under stressful conditions. Meet patient/family individualized psycho social needs. Requires hand-eye coordination and manual dexterity.Environmental/Working Conditions:
Works primarily in an office setting. If required to make patient visits, may work in patients home in various conditions Possible exposure to blood borne pathogens, bodily fluids and infectious diseases. Some exposure to unpleasant weather; PRN emergency call. Ability to travel locally and work a flexible schedule.
