Horizon Home Care and Hospice, located in Milwaukee, WI allows our RNs the opportunity to bring quality of life to the end of life. For hospice patients and their families, it is important for them to know they have round-the-clock support. When symptom control issues arise or intensive care needs are required, we offer an RN to step in, no matter what time of day.

That is where our Continuous Care program comes in. You have the ability to create a positive hospice experience during a challenging time. Provide that essential end of life care, when it matters most.  It is a meaningful responsibility and, for many, a calling.  There is no job more rewarding.

We have both part-time and full-time Hospice RN Continuous Care opportunities available for both 1st shift (8am - 8pm) or 3rd shift (8pm - 8am), including a holiday rotation and every other weekend. Positions are eligible for generous benefits including health, dental, and vision insurance, retirement savings plan w/ employer match, paid time off, life insurance, tuition reimbursement, mileage reimbursement, and more!

Qualifications include a degree from an Accredited School of Nursing, WI RN or LPN license, and at least one year of previous nursing experience. While a RN is preferred, we will most certainly consider someone with a LPN and strong LPN experience.  Since these positions travel throughout Milwaukee, Waukesha, Ozaukee, and Washington County, you must also have a valid WI driver's license, auto insurance, and a reliable vehicle.
