We are seeking to hire a full-time Nurse Care Manager (NCM) to help us launch and manage our new buprenorphine treatment program. The NCM will work with primary care providers (physicians, nurse practitioners) to provide team-based care, and manage the care and coordination of patients being treated for an opioid use disorder.

The NCM (with the support of the team) will screen and assess patients eligible for treatment, provide patient care management, medication management, motivational counseling, follow-up care, and monitoring, and refer for more intensive treatment as necessary. The NCM is the point person that will have the most contact with the patients, and will be in constant contact with the primary care providers in order to communicate patients’ needs and shape patients' care.

Responsibilities Include:

  • Provide direct patient care
  • Assist in the development of initiative-related clinical and administrative policies and procedure, workflows, electronic health record template development, and internal and external referral process
  • Conduct outreach (meetings, events, materials distribution, etc.) in venues or neighborhoods where there is unmet need for buprenorphine treatment and identifying individuals who may benefit from buprenorphine treatment
  • Screen and assess patients including obtaining patient demographic information and consents
  • Manage induction, stabilization, maintenance, and relapse prevention in coordination with primary care providers. Manage patients under the guidance of the waivered prescriber with close clinical follow-up and ongoing communication with the waivered prescriber by telephone, EMR, and team meetings
  • Coordinate ongoing follow-up care, close telephone monitoring, relapse prevention, and support for patient self-management
  • Collaborate as needed with other care staff, including physicians, social worker/counselors, psychiatrists, pharmacy, and specialty care providers to whom the patients are referred
  • Coordinate between prescriber, pharmacy, and patient, by assisting with prescription processing and refills, prior authorizations, insurance issues
  • Refer patients for more intensive substance use disorder treatment or other services, such as harm reduction services, as necessary
  • Conduct re-engagement efforts when patients miss medical appointments without prior notice to the medical provider or program staff
  • Submit monthly reports on program deliverables to Public Health Solutions as necessary
  • Other duties as assigned by the Chief Medical Officer

Education and Experience :

  • Licensed to practice as a Registered Nurse (RN) in New York
  • Minimum of 3 - 5 years of experience as a professional nurse
  • Minimum of 2 years of experience working with underserved and/or substance use populations
  • Bilingual in English and Spanish is desirable, but not required
  • Prior experience conducting individual patient education and counselling sessions
  • Demonstrated ability to work collaboratively in a team environment
  • Demonstrated computer literacy in Microsoft and web-based applications
  • Excellent verbal and written communication skills
  • Excellent interpersonal and organizational skills, including problem solving with a team
  • Knowledge of community resources; demonstrated ability to network and build strong relationships with community organizations serving priority populations as identified by the agency and/ or funder
  • Demonstrated ability to working with patients of diverse backgrounds, underserved communities, and co-morbidities
