Community Care Nurse Manager is accountable to the Service Chief of Community Care and is responsible for the successful operations of the Community Care Division, and assures that the program meets its goals through direct collaboration with clinicians from primary, specialty care, and ancillary care Divisions (Operative Care, Hospital and Specialty Medicine, Primary Care, Rehab & Long-Term Care, Mental Health, Imaging, Laboratory, Prosthetics, Pharmacy, etc.), and with administrative programs (Business Office, Fiscal Services, etc.).
The NM leads and serves as clinical resource to the team of RNs within Community Care.
- Collaborates with other services throughout the entire health care system and its satellite facilities to coordinate provision of non-VA care alternatives when needed.
- Responsible for overall management of the Community Care Service, providing leadership to the nursing staff of the Community Care Service and ensuring ethical, efficient, high quality, cost-effective, and coordinated services. As the leader of the Community Care nursing team, the incumbent directs strategic planning, program development, budget management, and quality improvement activities.
- Has shared accountability for the successful operations of the Community Care Service, and assures that the program meets its goals through direct collaboration with Veteran patients, community vendors, and other VA clinical staff from all medical services.
- Responsible for direct oversight of the clinical care of the diverse patient population that is engaged in non-VA care each year. Direct oversight includes care coordination management activities necessary to purchase patient care in the community; balancing clinical, administrative, and financial priorities in a way that maximizes the patient experience and VA's resources. This includes, but is not limited to, identification of patients appropriate for non-VA care services; triage of non-VA care consults received; coordination of care for the patient with the community vendor; conducting make/buy analysis; directing authorization staff in order to maintain fiscal efficiency; and integrating the patient back into the VA system when the non-VA care episode is completed. In addition, the incumbent frequently acts on behalf of VANCHCS with other VISN21 sites to determine availability of clinical services, ensure proper hand-off of patients, participate in collaborative care coordination planning, utilization review activities and other related care coordination activities.
- Maintains a close working relationship with key medical, administrative and clinical staff at each VISN 21 VA facility as well as VANCHCS' affiliates.
- Responsible for activating and monitoring Process Improvement programs according to organizational policy, regulatory standards, and determined need. (S)He is responsible for updating and facilitating implementation of hospital policies regarding non-VA Care activities. Incumbent ensures programmatic compliance with all standards, policies, directives, mandates, and regulations, including Joint Commission, VA's Non-VA Care directives, and other appropriate regulatory agency standards governing the management of non-VA care data requirements.
- Required to work with Service Chiefs, direct patient care providers, social workers, and other healthcare disciplines across VANCHCS on a regular basis in the course of ensuring quality non-VA episodes of care.
- Largely responsible for developing and meeting budgetary targets, making accurate financial projections, conducting make/buy analysis, monitoring and verifying expenditure of budgeted funds, and adhering to fiscal requirements. The incumbent is indirectly responsible for management of the medical center's entire Community Care budget, often requiring collaboration with clinical program staff that are outside the confines of the Community Care Service to accurately project and manage their portions of the overall Community Care budget.
- Works collaboratively with VANCHCS Fiscal Service & the Community Care Business Manager to ensure appropriate financial management of the Community Care resources.
- Responsible for the recruitment, selection, and training of high quality clinical & administrative staff to the program.
Incumbent sets performance goals for the team. He/she will assure that policies, practices and services are consistent with professional standards and with the mission and vision of VANCHCS, VISN, and VA Central Office Non-VA Care services.

He/she is responsible for initiating and participating in interdisciplinary projects and committees with the aim of improving outcomes in the management of complex patient care or the systems and process of the organization/service. The NM demonstrates exceptional communication skills and models transformational leadership skills. He/she is engaged and leads all labor management relations for Community Care Services.

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Preferred Experience:
  • One to three years successful nursing practice in a progressively more responsible leadership assignment.
  • 3 years of practice as a Manager/Director in a Health Care setting within the last 5 years.
  • Strong Fiscal and Budget working knowledge

  • Current proficiency in Basic Life Support (BLS) through the American Heart Association (Required prior to assuming clinical duties and must remain current -VHA Directive 11-77)
  • BSN and/or Master's degree from an accredited school of nursing recognized by the VA (Commission on Collegiate Nursing Education (CCNE) or The Accreditation Commission for Education in Nursing (ACEN).
  • Member of an appropriate professional nursing organization and seeks continuing education and professional development opportunities consistent with the annual learning plan.
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
  • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
  • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
  • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
  • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree.
