Community Health Nurse I-II (Health Care Program for Children and Youth with Special Needs)
The Health Care Program for Children and Youth with Special Health Care Needs (HCP) provides care coordination services, education, support, and home visits for children and youth with special health care needs. The HCP program is a voluntary medical/psychosocial case management program for children and youth, ages 0-21 who have, or are at risk for, physical, developmental, behavioral, or emotional special health care needs. There are no financial or diagnostic eligibility criteria. Services are provided at no cost to the family and the length of service is determined by the nurse and family, and guided by identified family goals. Professional work experience related to children with special health care needs or other community health, public health, or pediatric experience is preferred, but not required. This community health nurse works within a multi-disciplinary team to identify and address the medical and psychosocial needs of the child, youth, and family by connecting them with appropriate medical, specialty care, mental health, educational, family support, and basic needs resources/services. This Community Health Nurse will be assigned to the multidisciplinary community health services team and will work to provide individualized in-depth case management services to identify and address the medical and psychosocial needs of children, mothers, and families and to connect them with appropriate resources and services. This nurse will establish therapeutic relationships with each client and provide physical/educational assessments, health promotion, and health education, according to program guidelines. Additionally, this Community Health Nurse will participate in public health emergency response activities. Training will be provided.
The Community Health Nurse position is open as level I or II and will be based on experience and education. Salary will be commensurate with the job requirements, experience, and education.
