The RN II is a registered nurse with an Illinois State License.  The RN II, competent to proficient, provides nursing care to acute care patients while collaborating with the nursing care team and multidisciplinary team to advance the patient plan of care.  Nursing practices is guided by the American Nurses Association (ANA) Scope and Standards of Professional Nursing Practice and Code of Ethics.  The RN II provides leadership within the nursing care team to advance patient care and outcomes and improve the quality of nursing care provided by the organization.  The RN II uses past experiences and clinical knowledge to make judgments about appropriate care for patients.  Collaborates with the multidisciplinary team to create a comprehensive plan of care.

This position adheres to and is responsible for the safety standards of Centegra Health System including the completion of the mandatory organizational safety standards on an annual basis


Identification of population served by this position: Neonate through Geriatric as determined by department scope of service.



Application of the nursing process

  • Assessment: The RN II assesses the healthcare needs of patients/families by completing a physical and psychosocial assessment of patient/family condition.  Includes pertinent data in assessment.  Identifies and anticipates subtle changes in patient condition based on physical assessment and patient data. 

·        Demonstrates knowledge regarding the biological, psychosocial, ethnic and religious needs of the patient population, in addition to the age-specific learning needs, and incorporates those needs into the individualized plan of care.  Acts as a resource to the novice nurse to address individualized patient care.

  • Diagnosis: Analyzes assessment data to determine potential nursing diagnoses/patient problems. 
  • Outcomes Identification and Planning:  Collaborates with patients/families, nursing care team, and multidisciplinary team to identify expected outcomes/goals of care for each patient and contributes to care planning to achieve individualized patient goals.  Recognizes patient/family as an integral member of the healthcare team.  Shows ability to independently problem solve and shares perspective with the multidisciplinary team.

·        Implementation:  RN II implements the identified plan of care by providing evidence-based interventions. Efficiently coordinates patient care for assigned patient load and support others to provide care to the larger team of patients.     The RN II identifies elements of care that are appropriate to delegate to unlicensed assistive personnel and provides follow up to ensure all delegated care is provided.  Provides detailed instruction and feedback to unlicensed assistive personnel.    Coordinates patient care needs, including discharge planning, with appropriate departments and identifies patient/family discharge needs. 

  • Evaluation:  Evaluates effectiveness of plan of care by assessing patient/family response to interventions.  Collaborates with nursing care team and multidisciplinary team to alter plan of care as needed based on patient response.


Professional Performance

  • Practices ethically and maintains confidentiality of patient information.  Identifies situations that require ethics consultation/review.  Reports safety and ethical concerns using the organizational processes for reporting.  Able to articulate complex patient issues and contribute to ethical evaluation of a case.
  • Maintains knowledge and competency to reflect current nursing practice including competent level of knowledge of the nursing process.  Completes mandatory education as identified by Centegra Health System.  Evaluates own practice and establishes professional development goal(s) every year and develops a professional development plan to achieve goal(s).  Willingly gives and receives feedback related to nursing practice in the practice environment.  Identifies opportunities for process improvement based on data and experience and suggests solutions.
  • Communicates clearly and efficiently to members of the nursing care team and multidisciplinary team using established communication tools to support safe and effective handoffs, transfer of information, and delegation to other members of the care team.  Identifies critical issues and is able to prioritize the need to communicate critical and non-critical patient care information.  Effectively documents care plan and patient care. 
  • Demonstrates leadership in the care of assigned patients and acts as a clinical coach/preceptor to others.  Maintains responsibility for delegated tasks and provides feedback to team members.  Collaborates with patient/family and healthcare team.
  • Demonstrates financial stewardship by using resources wisely and in an environmentally safe and healthy manner.  Identifies opportunities to improve resources or resource utilization. Demonstrates understanding of effective staffing principles.

·       Demonstrates independence in clinical practice.

·        Demonstrates compliance with institutional and department policies and procedures using available resources.

Knowledge, Skill, and Physical Abilities Required

Minimum Competency



18 months of experience as a Registered Nurse

Greater than 2 years of full-time experience as a registered nurse


BSN preferred, ADN minimal requirement

BSN, ADN candidates must agree to complete BSN within 5 years of hire date


BLS (specialty areas may define additional certification requirements)

BLS and ACLS (specialty areas may define additional certification requirements)


National certification in area of specialty (ANCC recognized certification)


Illinois Registered Professional Nursing License

Illinois Registered Professional Nursing License

Physical Ability to perform functions of the job 

Includes lifting  up to 40 pounds and ability to stand/walk for prolonged periods of time
