Position Summary: The Breast Health Nurse Navigator participates in assessing, planning, implementing and evaluating health services provided by the Breast Health Program. Acts as a resource for patients and their families confronting a breast cancer diagnosis and to the community in providing breast health awareness, proper screening and risk behavior modification guidance. This individual is responsible for coordinating and conducting health promotion and awareness programs in the community, supporting community mammography screening programs and coordinating the care needs of women with suspicious diagnostic breast imaging results and diagnosed breast cancer patients. Through the use of education and assessment tools, the Breast Health Nurse Navigator will serve as a central point of contact for all patients confronted with diseases of the breast facilitating patient access to services provided within the HRHS health system, resources available through the community or service agencies, providing education regarding breast cancer treatment, symptom management and long-term survival issues. The Breast Health Nurse Navigator will work collaboratively to coordinate patient care needs across the continuum, providing a central point of contact for the patient and clinicians involved in the care regardless of setting. Will serve as the facilitator for breast education programs, support groups and other patient related support programs.
Recruitment Requirements: Graduate of an accredited School of Nursing Current licensure to practice as a Registered Nurse in the State of Pennsylvania Bachelors Degree in Nursing (within 6 months) Minimum of 5 years experience in Oncology Nursing Specialty certification OCN/AOCN and Patient/Nurse Navigation preferred Certification as Patient/Nurse Navigator required within first 2 years of employment BLS certification required Membership in the Oncology Nursing Society, or similar Professional organization is preferred Excellent Written and Oral Communication Skills, Effective Public speaking and presentation skills required Proficient in use of personal computer programs including word processing and data management programs preferred
