The Clinical Outcomes Sepsis Coordinator is responsible for the coordination, implementation, improvement, and successful achievement of CMS Hospital Quality Alliance (HQA) Metrics compliance as well as other publicly reported measures as determined appropriate. Participates in physician and clinician collaboration as well as liaising between physicians, nurses, administration, patients, and external bodies regarding Sepsis treatment plan; development and implementation of specific tools for diagnoses; data abstraction and monitoring of metrics data. Responsible for quality improvement aspects such as data management, analysis, improvement strategies, and presentation to applicable departments, committees, or groups. Ability to establish good working relationships with physicians and staff.  Participates in Performance Improvement activities to assure continuous Quality Improvement. Promotes a supportive, team-work oriented environment.  Fulfills departmental responsibilities with the long-range plan, mission, vision, and values at Horizon.

  • Preferred Bachelors of Nursing (BSN) completed or in progress

  • Previous Nursing experience on an Emergency Department (ED) or Critical Care/ Intensive Care (ICU/CCU) Unit treating with Sepsis Patients

  • Familiarity with clinical quality data, metrics, and reporting. Must be proficient with Excel. Ideal candidate will have experience with use of HCA's clinical and reporting software.
