Renders advanced nursing practice services to all ages under a collaborative agreement with a designated PHMG physician.  Responsibilities include: assessment, diagnoses, treatment and when necessary referral to others for further care.  Will manage his/her own panel of patients in collaboration with a physician.  Collects patient health data, prioritized by the patient’s condition or needs, utilizing appropriate assessment techniques while involving the patient, family and other health care providers as indicated.  Ensures systematic, ongoing data collection with the relevant documentation, in accordance with the PHMG standards, the State of Wisconsin, PHMG Scope of Practice and Collaborative Agreement with the Nurse Practitioner.
