RN MDS Coordinator, PRN, 8a-4:30p
Under the direct supervision of the Director of Nursing, the MDS/Care Plan Coordinator will coordinate and supervise the timely completion of the MDS and implementation of the nursing care plan for assigned residents of the geriatric resident population. Operates under the department philosophy, standards, and policies and will assist with applying the nursing process under the supervision of the D.O.N. Must be able function in the capacity of a RN II. Assists in evaluating resident care; revises and updates nursing care and treatment plans; participates in resident care and team conferences and assists in establishing priorities in resident care. Assists with monitoring and evaluating of quality and appropriateness of resident care. Coordinates and maintains resident care plans so they are current and reflective of resident. Communicates care plans to staff, residents and families. Assists Director of Nursing with reporting significant changes to resident¿s physician. Efficiently communicates with residents, staff, families and visitors. After completion of MDS, gives MDS to Director of Nursing for appropriate signature. Invites residents and families to care plan conferences. Recognizes and responds to health needs of residents. Makes prompt and accurate nursing care judgments. Coordinates and maintains completion of the RAI, quarterly reviews and specialized assessments. Prepares care plan calendar monthly, keeps calendar current. Complies with blood borne pathogen procedures, annual employee health assessment and with standard precautions. Conducts interdisciplinary care plan meetings. Participates in on-thejob training of personnel and in teaching and reinforcing teaching plan to resident and family. Provides feedback to C.N.A¿s regarding performance and participates in performance review process with Nursing Department managers. Assists other members of the health care team as requested. Performs other related duties as required. MINIMUM QUALIFICATIONS: Current licensure as a Registered Nurse in the state of Georgia. Must have 3 years previous MDS experience. BLS, ACLS, and Healthcare Provider certifications required. PHYSICAL REQUIREMENTS (Medium): 20-50 lbs; 0-33% of the work day (occasionally); 11-25 lbs, 34-66% of the workday (frequently); 01-10 lbs, 67-100% of the workday (constantly); Lifting 50 lbs max; Carrying of objects up to 25 lbs; Occasional to frequent standing & walking, Occasional sitting, Close eye work (computers, typing, reading, writing), Physical demands may vary depending on assigned work area and work tasks. ENVIRONMENTAL FACTORS: Factors affecting environment conditions may vary depending on the assigned work area and tasks. Environmental exposures include, but are not limited to: Blood-borne pathogen exposure Bio-hazardous waste Chemicals/gases/fumes/vapors Communicable diseases Electrical shock, Floor Surfaces, Hot/Cold Temperatures, Indoor/Outdoor conditions, Latex, Lighting, Patient care/handling injuries, Radiation, Shift work, Travel may be required. Use of personal protective equipment, including respirators, environmental conditions may vary depending on assigned work area and work tasks.
