Position Summary and Key Responsibilities

RN Case Managers at City of Hopework collaboratively with world class interdisciplinary team members. Patient safety is always a priority. Patient care needs are complex, however there is a strong commitment to providing the resources necessary to facilitate timely care coordination. A collaborative approach to problem solving, communication, and shared accountability make this department unique. This position reports to the Director of Case Management who believes in developing the team to work to the top of their license and empower them to make decisions that directly impact the way that we do our work.

Minimum Education and Skills Required for Consideration:

  • Bachelor of Science in Nursing (BSN) degree required

  • Three years current case management experience in an acute care setting is preferred

  • Current California Registered Nurse License

  • Current BLS Certification through the American Heart Association

  • Preferred Certification: OCN, CCM
