Job Description
The wraparound facilitator is responsible for teaching and supporting at-risk families to learn and use skills to develop their own plans and access their own resources. The position works closely with families and community partners to identify strengths and needs across all relevant life domains, building and strengthening their natural support network. The wraparound facilitator develops a partnership relationship with the family and community resources to address and work through challenges to make changes in lives and 'move families up!' The facilitator will assess the families' readiness to change, foster their engagement in self-help activities, and coordinate referrals to appropriate services within the broader community.


Education: Bachelor’s degree in health science field.

Licenses/Certifications:  None.

  • Three (3) years’ experience in a social service or health care- related role which includes individual or group counseling in behavioral change, motivational interviewing and community engagement. Effective oral and written communication skills.
  • Demonstrated ability to assess families' needs and readiness to change as well as develop appropriate goals for long-term success.
  • Knowledge of motivational techniques, adult learning and behavioral interviewing required.
  • Possess an understanding of child and adolescent development.
  • Bi-lingual ability preferred.
  • Skilled in reflective listening and motivational interviewing as well as demonstrate excellent oral and written communication skills.
  • Strong organizational and ability to work effectively with a wide range of constitutes in a diverse community.  Knowledge of community resources and an ability to engage others.
  • Strong interpersonal skills and sensitive to patient needs and concerns.

  • The wraparound facilitator is responsible for reaching out to build and maintain effective individual family and community relationships.
  • Performs individual child, youth and family strengths and needs-based planning across all relevant life domains.
  • Provides ongoing coaching, teaching and social support to help families build self-efficacy skills and prioritize, establish and achieve personal social determinant of health goals.
  • Coordinates services across a multitude of systems and community resources.
  • Using motivational interviewing techniques assesses patient readiness to change behaviors. In partnership with the family, develops, implements, monitors and documents the family's plan to improve social determinant of health changes. Sets realistic goals in relation to level of readiness and helps remove potential/existing obstacles.
  • Collaborates closely with a team of family members, material supports and professionals who develop and carry out individualized plans.  The coordinator has an understanding and experience with different systems, including schools and social service agencies.
  • Monitors goal progress and provides ongoing support through telephonic, electronic or in-person encounters. Promotes use of secure electronic correspondence with families.
  • Identifies and assembles relevant and community-based resources including those that may support members of our culturally diverse patient population.
  • Gathers data, develops required utilization reports and participates in the assessment of the program.
  • Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
  • Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.
  • Other duties as assigned.
