Job Description
Responsible for providing direct patient care in the Hospice Inpatient Unit or Home Hospice environment under the direction of a Registered Nurse, utilizing the clinical and cognitive skills specified in the Hospice LVN Skills Inventory record, including specialized and expanded skills, exhibiting broad nursing knowledge and an ability to manage and communicate effectively with the interdisciplinary healthcare team, patient/family and referring agency.

MINIMUM QUALIFICATIONS:

Education:
  • Graduate from an accredited school of vocational nursing or licensed practical nursing.

Licenses/Certifications:
  • Current, unrestricted State of Texas license/permit to practice as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) required.  Certified in Basic Life Support required.

Experience / Knowledge / Skills:
  • Minimum of two (2) years acute care hospital, hospice, or long term care facility experience as an LVN; has acquired skills related to specific specialties.
  • Ability to travel to patient homes or inpatient facilities as assigned.

Principle Accountabilities:
  • Under the direction of the RN, assist in identifying the patient’s physical, psycho social, and environmental needs as evidenced by documentation, clinical record, team report, and evaluations.
  • Collaborates with the multidisciplinary team while making clinical decisions in the delivery of patient care; Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.
  • Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
  • Provide effective communication to patient/family, team members, and other health care professionals asevidenced by clinical notes, case conferences, communication notes, and evaluations.
  • Monitor assigned cases to ensure compliance with requirements of third party payor.
  • Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
  • Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.
