Licensed Practical Nurse (Community Resource & Referral Center (CRRC)) - (Primary Care)
The Community Resource and Referral Center (CRRC) is located in Portland, OR and provides services for Veterans and their families who are homeless or are at risk of becoming homeless. The incumbent is a member of the Patient Aligned Care Team (PACT) and uses processes and evidence-based practice to collaborate with the PACT team in developing the patient-driven care plan for life. The incumbent is responsible for collaborating with services, internal and external to the VA, to facilitate care transition in order to effectively meet the patient's needs and provide patient and family health education with a focus on self-management, prevention and wellness based on the patient's goals. Candidates should possess varied and extensive experience in medical and mental health practices. The position will require significant coordination of care and community outreach to individuals and organizations outside the VA.
The LPN/LVN can be expected to operate all administrative and medical equipment safely and correctly in a Primary Care Clinic. Demonstrate excellent listening and comprehension skills. Can assist in deescalating clients and in service recovery. Is responsible for providing focused nursing assessments and administrative support. Observe changes in normal to abnormal, collects data and client history as ordered. Contribute to the assessment of single system reviews. Document the data collected in a systematic way that contributes to the medical and nursing knowledge of the primary care team. Will assess single system health issues and defer multi-system assessments to the Teamlet RN. The LPN/LVN will provide triage for simple to moderate patient calls for medical or nursing help, and work with groups of clients in educational settings. The incumbent completes the Health Promotion/Clinical Reminders as appropriate and documents accordingly in CPRS. Offer concern for changes in plan of care to other team members. Know the differences between normal and abnormal at the individual level. Assist with medical procedures, time outs, set up trays for specific types of exams. Assists with classes and group appointments. Complete a system of nursing tasks that are age/era appropriate for the veteran and orders that include but are not limited to the following; injections, giving medications and vaccines, airway and breathing treatments as ordered, EKG's, bladder scans, pulse oximetry, Snellen Chart eye exams, wraps, catheterizations, wound observations and documentation, Dopplers of legs, foot exams, skin observations, women's health exams, hygiene and simple nutrition teaching, medication teaching, suture removal, glucose meter reading and transmission to the record. Demonstrates flexibility in taking clinic assignments. Complete a system of tasks that are specific to Primary Care services which includes but is not limited to; vital signs collection, completion and interpretation, specimen collections and phlebotomyas prescribed by policy, administers and participates in team assignments, determines a medical emergency and assists with care provided.
PREFERRED CANDIDATE: Minimum of 2 years of nursing experience with knowledge of sterile/procedural care, scheduling, excellent customer service and multi-disciplinary services. Experience working with homeless or other vulnerable populations with an emphasis in mental health is desirable.
WORK SCHEDULE:Monday through Saturday; varying tours

GRADE DETERMINATIONS: In addition to the basic requirements, the following qualification criteria must be met for each grade. The candidate's qualifications must clearly demonstrate the level of competence required for the grade as provided below in specialized experience:
SPECIALIZED EXPERIENCE FOR GS-3 LEVEL: None beyond the basic requirements.
SPECIALIZED EXPERIENCE FOR GS-4 LEVEL: Employees may be appointed at or advanced to GS-4 if they meet any of the criteria: demonstrated six months of qualifying experience as an LPN or LVN; orGraduation from an approved schooland one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures; or graduation from an approved school of at least 24 months duration.LPNs at the GS-4 grade level perform all duties expected of employees at the GS-3 grade level; however, they are expected to exercise greater judgment, require less supervision, and to operate in accordance with all established policies, procedure, and techniques.
SPECIALIZED EXPERIENCE FOR GS-5 LEVEL: Demonstrated knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems. Works with the RN, ODand/or MD/DO to appropriately orient/train less experienced LPNs/Ophthalmic Techand/orOptical Techs/Health Technicians (HTs)in relation to acceptable standards of practice in promoting optimal patient care delivery. Provides effective education to patients and/or family members in relation to common disease processes, medication, and /or prescribed treatment regimes. Demonstrated ability to serve as a responsible member of the nursing team and interact in an appropriate manner with patients, family members, professional and other supportive personnel involved in the delivery of patient care, incorporating acceptable, established customer service standards into practice. Knowledge and skill sufficient to prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous, intramuscular, and/or intravenous medications as permitted by approved local facility policies and procedures. Observation and documentation will include patient's response to medication administered and the reporting of any noted change in patient's condition to RN, OD or MD/DO.
SPECIALIZED EXPERIENCE FOR GS-6 LEVEL: Requires a minimumof at least one (1) year of additional qualifying experience at the GS-5 level or equivalent:Technically proficient in initiating, performing and completing assigned duties in providing care to variable patient populations.Knowledge and ability to appropriately carry out assigned patient care based on the patients' conditions; to use judgment in selecting the appropriate order and sequence of procedures and treatments; and to accurately recognize, report and record relevant] patient information. Completed work should need only a general review by a registered nurse (RN) or physician (MD/DO) forappropriateness and conformity with established policies/procedures.Ability to observe, identify and respond to the patient's needs for care, including medication,equipment-assisted care and patient/family education. In organizing and delivering care, the LPN/LVN recognizes and considersemotional, cultural, spiritual, socio-economic, and age-related factors.Prepares and administers prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous) and performs treatments according to established policies/procedures. Observes for physical and/or emotional changes in patient's condition from prescribed medications/treatments, promptly and accurately documenting noted changes, and reporting any deviations from normal to RN or MD/DO.Knowledge and ability to recognize urgent or emergent patient care situations, seek assistance of the RN and/or MD/DO, and initiate appropriate emergency interventions as directed.Knowledge and understanding of human behavior, patient motivations and reactions to situations, and ability to appropriately utilize this knowledge in working effectively with patients, family members, and other staff.
