Position Summary:
We have an exciting opportunity to join our team as a Clinical Coordinator.

Position Summary:Coordinates and performs diagnostic, therapeutic and preventive patient care activities. Provides for and assures comprehensive nursing care through use of nursing process and appropriate standards of care.
The pharmacist / nurse case manager will work under the supervision of the Pharm D to be the primary day-to-day contact for the Pharmacy Benefit Management contractor for troubleshooting and problem resolution. S/he will work closely with the case management team, nursing staff, provider staff and administrative staff on operations issues related to member prescriptions. S/he will be the first contact for calls from external pharmacists, members and providers related to member prescriptions.

Job Responsibilities:
  • Provides for educational needs of self, patients, and administrative staff. Participates in development and upgrading of interdisciplinary and nursing standards and patient/family education materials and programs as relevant to expertise and practice. Provides nursing consultation and educational programs to nursing and medical staff as requested and indicated. Participates in outside professional and community activities as appropriate and requested. Participates in hospital and departmental committees as appropriate and as requested. Monitors and trends cost effectiveness of position, particularly as it relates to efficiency and volume of service or practice.Helps educate staff and acts as a resource for clinical questions from staff.
  • Provides for educational needs of self, patients, and administrative staff. Maintains current expertise in area of practice. Participates in hospital, nursing, State and JCAHO continuing education programs as required and as indicated. Participates in development and upgrading of interdisciplinary and nursing standards and patient/family education materials and programs as relevant to expertise and practice. Provides nursing consultation and educational programs to nursing and medical staff as requested and indicated.
  • Provides patient care to the Division of Vascular Surgery patients, in conjunction with the development of establishing and building his/her own practice. Evaluates patients treatment and health care plan for effectiveness and modifies per clinical standards and practice protocols. Documents all assessments, findings, progress and care provided in patients medical record. Works collaboratively with other disciplines in developing and implementing the therapeutic plan of care. Assists physician in maintaining timely and appropriate communication with other disciplines and ancillary services. Triages patient calls to assess if they are emergent.
  • PATIENT CARE. Provides patient care to the Division of Vascular Surgery patients, in conjunction with the development of establishing and building his/her own practice. Coordinates and schedules patients for the Chairman (office, surgery, other). Orders non-invasive and diagnostic testing, differentiates between normal/abnormal findings and refers abnormalities to supervising physician per protocol. Participates in provision of health care, teaching, counseling and treatment to patients in outpatient setting and refers significant clinical issues per protocol to supervising physician. Prescribes medical equipment, devices, and home health services per practice protocol. Prescribes and writes prescriptions for medication according to New York State law (including controlled substances provided DEA number has been issued) per protocol.
  • MEDICAL HISTORIES. Performs medical histories and physical examinations. Elicits a detailed and accurate medical history from patient and performs appropriate physical examinations. Records appropriate data in medical charts Organizes and presents pertinent (both normal and abnormal) data in a manner meaningful to physician for his/her medical diagnosis and treatment.
