Position Summary:
We have an exciting opportunity to join our team as a Nurse Practitioner-Affiliation.

In this role, the successful candidate The Nurse Practitioner (NP) will provide high-quality diabetes care to new and existing patients in the NYC Health + Hospitals/Bellevue Endocrine Clinic and Adult Primary Care Center (APCC). The NP will report to the Bellevue Endocrine Section Chief and to the APCCs director of Diabetes Care. They will work 9 clinical sessions and one administrative session/week.

Job Responsibilities:
  • Work collaboratively with colleagues in the APCCs A1C (Diabetes) Program
  • Perform a comprehensive evaluation of patients diabetes
  • Order and review laboratory, radiographic and ancillary studies
  • Make referrals to specialists, as indicated
  • Document diabetes evaluation in the QuadraMed Electronic Health Record (EHR)
  • Communication/coordination of results with PCP and patients, as necessary
  • Assist with medication refills, patient forms and laboratory re-orders pertaining to diabetes care
  • Participate in inter-disciplinary care management of high risk diabetic patients
  • Participate in quality improvement activities
  • Review charts/reports for diabetes collaborative care and diabetes population health management

Minimum Qualifications:
To qualify you must have a Licensure: Current registration as Professional Nurse in New York and Registration as Nurse Practitioner in New
York;Education: Master's Degree in Nursing;Establishes a written Collaborative Practice Agreement with an NYUHC
attending physician;Credentialed through the Credentialing Committee of the Medical Board with Delineation of Privilege
approved with role specific treatment privileges;Certification required (as soon as feasible following graduation from NP
graduate program) by ANCC or Board Certification from other specialty organization that certifies Nurse Practitioners in a
clinical area of practice.ACLS, PALS, NRP Provider;(Neonatal Resuscitation Program), NALS per delineation of
privileges; PRI Certification/Screening is optional;Competence: At least five years of clinical nursing practice in the care
of the patient population served; ability to apply nursing process toward achievement of specific outcomes in an
organized fashion, which addresses patient/family safety and satisfaction. Previous experience as a nurse practitioner is
preferred. Evidence of excellent interpersonal skills, effective communication skills, creative problem solving and
excellent critical thinking and leadership; Previously demonstrated ability to facilitate group processes and work
cohesively and collaboratively as member of the interdisciplinary team; Effective change agent..

Required Licenses: Nurse Practitioner-NYS

Preferred Qualifications:
Certified Diabetes Educator or interest in obtaining CDE certification preferred.
