What You Will Do:

UM Baltimore Washington Medical Center is currently seeking a High Risk Care Coordinator (RN) to join our Care Management Team working with our Skilled Nursing Facility.

The High Risk Care Coordinator is responsible for seeking and implementing interventions, methodologies, and best practices for readmission reduction. Using an evidence-based tool, the High Risk Care Coordinator identifies and assess the patient for priority needs, key areas for needed support and services that contribute to readmissions; develop care plans on high risk patients and when appropriate refers for services to assist in navigating barriers to care. Will effectively collaborate and communicate with internal and external customers to maximize the patient’s potential for a healthy lifestyle evidenced in reduced Emergency Department visits and readmissions to the hospital. Effective data collection and analysis is an intricate responsibility of this role. The position is goal-oriented with a high level of accountability correlated to the reduction of readmissions, continuum of care, quality of care, and patient satisfaction.

Responsibilities include:
  • Readmissions and chart analysis as well as Breakdowns, date manipulation and reporting.
  • This position will responsible for working with several other Hospital Facilities in the Greater Chesapeak region to set goals, improve processes, and run quarterly meetings.
  • Experience with CRISP is necessary as this position will be closely working with this Health Exchange

  • What You Need to Be Successful:
    • Bachelor’s degree in nursing or related field required, Master’s degree preferred. Qualified applicants currently enrolled in a BSN program scheduled to graduate within 18 months of hire or transfer into the position will be considered.
    • Current Maryland State Licensure as a Registered Nurse (RN) .
    • Three (3) years of experience in discharge planning, case management or disease management required.
    • Strong skills in patient education, coordination of discharge planning, and post discharge follow-up.
    • Proficiency in use and interpretation of word, excel and other data collection programs.
    • Skills: Must be proficient in effective verbal and written communication; have strong organizational and problem-solving skills, the ability to perform multiple tasks with interruptions, and to manage time effectively in order to achieve simultaneous goals. Must demonstrate a commitment to goals, teamwork, and standards of excellence. .
