The purpose of the pharmacist position is to review patient medication use on a specific unit(s), make recommendations for improved therapies, educate patients and/or caregivers on patient(s) probable discharge medications, serve as an advocate for pharmacy to facilitate nursing on pharmaceutical questions, dosing, kinetic dosing of antibiotics, etc.; and/or therapeutic interchanges as part of a interdisciplinary team in caring for patient(s). Additionally, the clinical pharmacist will provide medication education to nursing and physician personnel as required. Response to medical emergencies is also a consideration of their duties.

Compounds and dispenses prescribed drugs in a healthcare facility environment. Conducts drug utilization review studies. Also responsible for transferring prescriptions, handling incoming physician calls, logging data into the computer, and Drug Utilization Review. Consults with medical practitioners on prescription orders, patient reactions, and errors. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks. A certain degree of creativity and latitude is required. Reports to Pharmacy Director.

Education: Bachelor of Science in Pharmacy or Doctor of Pharmacy degree (PharmD) from a college or a university accredited by the American Association of Colleges of Pharmacy.

Licensure / Certification / Registration: BLS, ACLS-preferred, Pharmacy License (RPh) in the State of Texas, Board Certification or eligibility within 1 year.

PGY-1 Residency in acute care setting. Minimum one year related experience preferred. Full understanding of Woodland Heights Medical Center department of pharmacy dispensing functions (up to 6 months assigned).
