  NYU Lutheran Augustana Center, provides care for people who are recovering from illness or injury but who are not able to return home safely. Specialized services include subacute and cardiac rehabilitation as well as ventilator care and extended care. Position Summary:
We have an exciting opportunity to join our team as a RN, Intake.

In this role, the successful candidate Under the direction of the Director of Patient Services (DPS), assesses and coordinates comprehensive patient evaluations, in medical facilities, rehab & skilled nursing facilities and community medical practices.
Job Responsibilities:
  • Accepts constructive criticism and makes appropriate changes.
  • Reports to duty as scheduled and on time.
  • Performs all other duties as assigned.
  • Participates in facility-wide in-service education and training programs.
  • Complies with all Federal and State regulatory requirements.
  • Maintains confidentiality of patient and program information.
  • Case conferences with the Director of Patient Services at a minimum of once per week, or more often depending upon the patient's status, to review patient's response to care and address any outstanding concerns.
  • Assists with orientation and/or guidance of new staff as needed.
  • Completes all documentation relating to patient care in compliance with program's policies and procedures.
  • Contacts physician regarding any discrepancies between orders and treatments.
  • Teaches and counsels the patient and family members regarding the home care needs and other related concerns of the patient at home.
  • Advises Clinical Manager and Operations Manager at least weekly of all NTUC referrals.
  • Maintains an accurate log of all referrals and referral data.
  • Coordinates care with other disciplines involved in the patient's plan of care, such as, social workers, nutritionists, physical therapists, physicians and other health care professionals.
  • Verifies all insurance information for accuracy. Obtains initial service authorizations from managed care organizations as necessary.
  • Conducts facility/community based comprehensive initial assessments including medical need, skilled services needs, safety, and psychosocial needs impacting patient's health and achievement of goals within twenty- four (24) hours or prior to discharge date.
  • Initiates patient plan of care as necessary. Documents same in EMR.
  • Collaborates with facility case managers, RN Managers and physicians to ensure triage and discharge planning are addressed in a timely manner.
  • Coordinates, triages and processes all referral information and admission orders using current software applications.
  • Learns and follows all program policies and procedures.

Minimum Qualifications:
To qualify you must be a graduate of an accredited Nursing School -BSN preferred
NYS-RN license with current registration
Minimum of two (2) years of CHHA and/or LTHHCP strongly preferred
Recent experience with OASIS documentation and physician orders preferred
Must possess strong assessment skills
BLS required
Ability to work in an environment with frequent interruptions.
Ability to communicate clearly and concisely.
Ability to handle multiple priorities at the same time.
Ability to set priorities among multiple requests.
Ability to establish priorities and coordinate work activities
Ability to establish and maintain effective working relationships with other employees, patient and patient families.
Ability to navigate multiple computer systems to document and coordinate patient care..
