||The Nurse Practitioner, in collaboration with the physicians, provides medical care and treatment for diabetes and endocrinology patients.
Essential Job Functions and Duties
Provide clinical support to physicians
Assess, plan, implement, and evaluate clinical needs of patients
Coordinate post-discharge follow-up care
Develop, coordinate and conduct educational and learning experiences
Maintains a comprehensive record of all active patients of the endocrinology service. This includes new consultations, and patients with follow-up visits
Conducts new patient evaluations and manages the care of established patients. Continuously reviews medical data with the attending physician, maintains accurate progress notes and engages patients in the educational process.
Performs comprehensive history and physical assessments in the clinic setting He/she provides for the continuity of patient care between the outpatient clinic and inpatient settings. Documents all patient data to ensure continuity of care.
Establishes medical and nursing diagnoses, prescribes, administers and evaluates pharmacological and other treatment regimens.
Identifies groups, families and individuals at risk and develops a plan to address those risks including, but not limited to, educational programs, screening programs and patient education materials.
Master’s Degree - Nurse Practitioner
Effective leadership and organization skills
Knowledge of EMR
Current State of Illinois Nursing License
Current Advanced Practice License
Academic preparation and/or expertise in program management and in the care of persons with a chronic disease
Certified Diabetes Educator (CDE) designation preferred
