  NYU Langone Hospital- Brooklyn is a full-service teaching hospital and Level I trauma center located in Sunset Park, Brooklyn. The medical center is central to a comprehensive network of affiliated ambulatory and outpatient practices, and serves as NYU Langone Medical Center's anchor for healthcare access, growth, and delivery in the entire borough.
Position Summary:
We have an exciting opportunity to join our team as a Nursing Quality Specialist - Stroke Center

In this role, The Stroke Quality Specialist facilitates the review of stroke related data and reporting of results to third parties, provides reinforcement of standards that support care for the adult stroke patient and coordinates community outreach and education activities.
Job Responsibilities:
  • QUALITY
    Facilitates concurrent chart audit for all stroke patients.
    Reports gaps in standard of care to responsible clinical and collaborate with clinicians to resolve gaps.
    Acts as a resource to Medical Center providers to work on stroke-related performance improvement activities in various departments.
    Facilitates a strong collaborative interdisciplinary approach to achieve optimal patient outcomes.
    Assists in the development and implementation of process standards for the delivery of stroke patient care.
    Assists the Core Stroke team in all specialty certification efforts and coordination.
    Assists in the development and implementation of action plans in response to internal and external benchmarking based on data gathered.
  • COMMUNITY OUTREACH & EDUCATION
    Being available to conduct stroke education in the community (Public health fairs, private company health screenings).
    Assisting in the provision of stroke education and awareness for providers in ancillary areas.
    Acts as liaison to the Nursing Education Department in identifying educational needs of nursing staff concerning stroke care issues
    Serves as support and resource for the education of other health professionals.
  • PATIENT CARE
    Conducting 90 day phone follow up with Primary Stroke Diagnosis patients for modified rankin score (mRS).
  • DATA
    Data abstraction for all Stroke diagnosis discharge cases at NYU Lutheran Medical Center.
    Stroke Data entry into UHC.
    Stroke Data entry into GWTG for all Stroke Codes.
    Stroke Data analysis.
    Stroke Scorecard for ED, Stroke Management and Stroke Steering Committee.
    Maintain Stroke Log in GWTG.
    Tracking and compilation of complication for EMS pre-notification.
    Tracking and compilation of complication data (IR, CEA, CAS, Mortality).

Minimum Qualifications:
NY State RN license required
BSN required
Experience: At least four years of clinical nursing practice in the care of patient population served, ability to apply nursing process toward achievement of specific outcomes in an organized fashion which addresses patient/family satisfaction;
Job Knowledge: Excellent chart review and data abstraction skills.
Evidence of excellent interpersonal skills, effective communication skills, and creative problem solving skills. Previous demonstration of ability to facilitate group processes and work cohesively with an interdisciplinary team. Membership in Certification: Relevant clinical certification/professional and/or specialty organizations.
BLS required
Physical stamina, manual dexterity, visual and aural acuity to perform responsibilities.
