The Social Worker provides psychosocial support and other Social Work services to Hospice patients and their families in the inpatient, nursing home and home care settings, in both pre and post bereavement phases of Hospice service. In this role, you will also serve as social work member of hospice interdisciplinary group IDG and as a liaison between hospice staff and inpatient and nursing home staffs. In addition, you will develop social work plans of care, target goals and make recommendations based on patient/family needs for assigned patients who are direct admissions to inpatient units or nursing home/hospice residents.

  • Graduate from the an accredited School of social work
  • New York State MSW licensure
  • Minimum of one year experience in Social Work in a healthcare setting
  • Knowledge of Netsmart EMR system preferred
  • Valid NYS Drivers license preferred.
