Care Coordinator – RN – Psych & Inpatient Medicine

Across NewYork-Presbyterian/Westchester Division, Care Coordinators are the guardians of continuity of care; they are the skilled clinicians leading the full spectrum of patient care, from pre-admission to discharge while ensuring quality outcomes. You'll manage complex cases and serve as a strong and knowledgeable liaison between Psychiatry and Inpatient Medicine Units, coordinating the efforts of social workers, specialists, PTs, OTs, cardiologists, intensivists and Psychiatrics – all in support of patients and their families. Direct all aspects of inpatient care, ensuring optimal length of stay. Anticipate outpatient service needs, setting appointments and consultations prior to discharge.

Preferred Criteria
    2 years' of case management/care coordination experience in an inpatient, HMO, insurance, or other outpatient setting 1 year of leadership experience in a healthcare settingAt least 1 year with complex medical patients & co-morbidity Discharge planning experience

Required Criteria
    Bachelors degree in Nursing (BSN)Current NYS Registered Nurse (RN) licensure3 to 5 years of health care experience in an acute care settingSuperior communication, demonstrated problem solving skills, strong business acumen, and interpersonal skills are critical
