  NYU Langone Hospital-Brooklyn is a full-service teaching hospital and Level I trauma center located in Sunset Park, Brooklyn. Position Summary:
We have an exciting opportunity to join our team as a Nurse Practitioner - Care Transitions.

In this role, the successful candidate shall be responsible for providing professional medical services in accordance with this Agreement and all applicable laws, rules, regulations and NYU Langone Health's policies and procedures, reporting to Director of Emergency Nursing and Care Transitions Supervisor in connection with these services. Provider shall provide the services described below during such hours each week as are assigned by the supervisor. In addition to other duties assigned by supervisor, the services include the following:

Job Responsibilities:
  • Provider shall provide direct clinical care to all patients assigned to Provider in accordance with applicable standards of care, NYU Langone Health's policies and procedures, as applicable.
  • Provider will be required to maintain certain good citizenship standards designed to optimize patient care and services. Such standards include but are not limited to the following: Accommodating walk in patients, arriving for office session on time and remaining throughout each session, engaging in teamwork with the support staff, following NYU Langone Health's procedures and practice guidelines, e.g., appropriate clinical charting with complete notes and up-to-date problem and medication lists, timely completion of encounter forms, completing peer chart audits, etc.
  • Provider will be required to participate on a rotating basis in the 24 hour on-call phone coverage arrangement that is unit/site-specific. Provider must make every reasonable effort to answer patients phone calls within 30 minutes on the day/evening Provider is on call and to submit all required documentation of the call which will be posted in the medical record.
  • Provider shall meet specialty and site-specific defined productivity expectations.
  • At all times, Provider shall accurately and timely complete any paperwork necessary to facilitate NYU Langone Health's billing and reimbursement.
  • Team Assignments: Health care delivery within NYU Langone Hospital-Brooklyn is based on interdisciplinary provider teams which provide collaborative care by physicians, as well as by nurse practitioners, physicians assistants, midwives and residents in training. Continuity of care is provided by the team over time, as well as between the outpatient and inpatient setting, through the unite/site specific mechanism that have been established and are in place for that purpose. Cross coverage for absent team members is a requirement. Cooperation and collaboration among team members is imperative.
  • Primary Care Provider (PCP) Communication: Every registered Family Health Center at NYU Langone patient has a PCP of record. As in most multi-specialty group practices, patients will be referred for a specialty consultation by that PCP who will have completed appropriate documentation of the consultation/referral. Communication, including through the proper completion of documentation, is an important member of the care that Providers will be providing to patients.
  • Provider shall respect cultural differences, religious differences, and the individuality of internal and external customers. Provider shall at all times project a professional image in demeanor and dress.
  • Commitment to compliance and quality improvement: Provider shall participate in compliance, quality management and performance improvement activities as requested, including but not limited to education and training efforts and audits of medical records.
  • Provider will make post discharge calls to high acuity patients.
  • Provider will provide assessments of pre-determined high risk individuals and identify specific barriers to compliance with treatment plan.
  • Establish short term goals and connect patient with needed post ED visit or inpatient care.

Minimum Qualifications:
  • Master's Degree in Advanced Practice Nursing.
  • Desire to work with marginalized populations.
  • Bi-lingual- English/Spanish or Chinese, Arabic, Russian or Yiddish depending on targeted patient population.
  • Computer literate (Microsoft Office), telephone, fax machine, and photocopy machine skills required.

Preferred Qualifications:
Experience working with patients who are diagnosed with mental health, substance abuse, and/or chronic health conditions in a variety of inpatient and outpatient settings, including home based visits
