Position Summary:
We have an exciting opportunity to join our team as a Nurse Practitioner- WOCN
Plans, develops, implements and evaluates the wound ostomy continence management program. Serves as an expert among the patients, physicians, families and ancillary departments in the care and management of this patient population. Functions independently as an advanced practice nurse within the parameters as defined by the Physician Collaborative agreement and practice privileges granted through Lutheran Medical Center. Assumes clinical, educational, research and performance management responsibilities.
Job Responsibilities:
  • Conducts consults on patients requiring complex wound, ostomy and continence management.
  • Conducts advanced health assessments and physical examinations necessary to develop an appropriate plan of care.
  • Implements treatment modalities and facilitates the wound, ostomy continence management plan of care in collaboration with Attending Physicians and according to evidence based practice and protocols
  • Prescribe and adjust treatments based on assessments or after discussion with collaborating physician.
  • Document treatment plan and progress toward outcomes in the progress notes and other required medical record documentation
  • Performs simple debridement, removal of staples and sutures in collaboration with attending physicians
  • Develop, implement and evaluate wound ostomy continence management protocols consistent with evidence based practices.
  • Provide education to the clinical team related to wound ostomy continence management.
  • Coordinates the quality monitoring and improvement activities related to wound ostomy continence management.
  • Standardize wound care products for use at LMC.
  • Act as liaison to LHC community

Minimum Qualifications:
Master's degree in Nursing, Adult or Family Nurse Practitioner
NYS RN license
NYS NP License with NP Certification
Wound Ostomy Continence Certification (WOCN)
Four years recent clinical nursing experience focusing on wound ostomy continence management
Qualified candidates must be able to effectively communicate with all levels of the organization.
