The LPN Transplant is an outpatient position focuses on overall health and population management of a cohort of patients. The LPN Transplant may work with pre transplant, post transplant, or living donor patients. Responsibilities include ordering and following labs, triaging phone calls, reviewing results, and coordinating information between the physician and patients.

MINIMUM QUALIFICATIONS: High School Diploma or GED required. Graduate of an accredited Licensed Practical Nursing Program. A minimum of four (4) years patient care experience preferred (within the last 5 years). Currently licensed as a Licensed Practical Nurse in the state of Georgia. Current BLS (CPR) certification before administering direct patient care.

PHYSICAL REQUIREMENTS (Medium): 20-50 lbs; 0-33% of the work day (occasionally); 11-25 lbs, 34-66% of the workday (frequently); 01-10 lbs, 67-100% of the workday (constantly); Lifting 50 lbs max; Carrying of objects up to 25 lbs; Occasional to frequent standing & walking, Occasional sitting, Close eye work (computers, typing, reading, writing), Physical demands may vary depending on assigned work area and work tasks.
