Director - Care Coordination

In this leadership role, you'll utilize your proven project management skills to build the institutional-wide care coordination strategy. You’ll leverage your experience in case management, care coordination and transitions of care to transform NYPs care coordination model. You'll lead the clinical and operational improvement activities across the continuum of care; facilitating integration of services; working effectively with other clinical leaders and senior administration; and ensuring organizational and departmental success. You'll report directly to the VP Care Coordination . We are seeking a stellar project director and change agent: a proven professional with the energy and drive to design and restructure NYPs care coordination model. This role supports the SVP/CNO and SVP/CMO in the planning, establishment and implementation of comparable standards of care coordination/management throughout the institution and Regional Hospitals ; evaluates outcomes, quality of practice and care and availability of resources. You'll also work closely with key leaders across nursing, social work, care coordination, patient case management and Information Technology, while building a bridge to the community.

Preferred Criteria

    Previous experience implementing care coordination/case management strategy within an academic medical center setting Certification
Required Criteria

    Masters degree Current NYS Registered Nurse (RN) licensure F ive to seven years of progressive case management leadership, with expertise in reimbursement methods, social work services, quality, and value-based purchasing NEA certification
