  NYU Langone Health Augustana Center provides care for people who are recovering from illness or injury but who are not able to return home safely. Specialized services include subacute and cardiac rehabilitation as well as ventilator care and extended care. Position Summary:
We have an exciting opportunity to join our team as a Registered Nurse- CHHA

In this role, the successful candidate Under the direction of the Clinical Services Supervisor (CSS) provides and coordinates comprehensive patient care, including, education of the patient and family, in order to maintain a maximum level of health.
Job Responsibilities:
  • Reports to duty as scheduled and on time.
  • Assume rotating weekend on-call responsibilities as assigned
  • Performs all other duties as assigned.
  • Complies with all Federal and State regulatory requirements.
  • Maintains confidentiality of patient and program information.
  • Case conferences with the Clinical Services Supervisor/Director at a minimum of once every two weeks or more often depending upon the patients status, to review patients response to care and address any outstanding concerns
  • Completes all documentation relating to patient care in compliance with programs policies and procedures
  • Contacts physician regarding any discrepancies between orders and treatments
  • Teaches, supervises and counsels the patient and family members regarding the nursing care needs and other related problems of the patient at home.
  • In accordance with physicians orders, observes signs/symptoms and reports to the physician reactions to treatments, including medications, changes in patients physical or emotional condition.
  • Coordinates care with other disciplines involved in the patients plan of care, such as, social workers, nutritionists, physical therapists, physicians and other health care professionals.
  • Provides quality nursing services, treatments, diagnostic and preventive procedures.
  • Conducts a follow up assessment within the last five (5) days of every sixty (60) days beginning with the start of care date, transfer, change in condition, or discharge and return during sixty (60) day episode.
  • Develops and implements the plan of care for the patient and completes no later than five (5) calendar days after the start of care date.
  • Conducts comprehensive initial assessments including environmental, safety, social, etc., impacting patients health and achievement of goals within twenty- four (24) hours or upon start of care date.
  • Learns and follows all program policies and procedures.

Minimum Qualifications:
To qualify you must be a graduate of an accredited school- BSN preferred.
NYS-RN license with current registration.
Current home health care experience preferred in CHHA/LTHHCP/LHCSA/MLTC.
Bilingual English/Cantonese required
