Registered Nurse-Patient Safety Manager
This is a permanent full time positionat the Veterans Affairs Medical Center (VAMC)located in Battle Creek, MI. The incumbent is assigned to the Office of the Director and reports directly to the Medical Center Director. The Registered Nurse (RN)in this positionfunctions as the Medical Center's lead manager, coordinator, and point of contact for patient safety policies and activities developed by the National Center for Patient Safety (NCPS). The incumbent is responsible for the development, maintenance, and operation of the Veterans Health Administration's (VHA)patient safety program at the medical center level. This includes working with all levels of staff within the medical center, including top-level management, patient safety teams, i.e. Root Cause Analysis (RCA), Healthcare Failure Mode Effects and Analysis (HFEMA), and other work groups including quality action teams. The incumbent is the Medical Center'skey person to help build and implement patient safety systems and promote the culture of safety within all healthcare settings across the MedicalCenter & OutpatientClinics and must have the attitude that building a culture of safety is paramount and the function upon which safe operations are based. Duties will include, but not limited to:
- Works,trains, and directs patient safety activities of similar personnel within the medical center i.e. Risk Manager, QI Facilitators, Nursing Educators, etc.
- Translates policies, theories, and strategies into action within the complex interrelationships that exist related to patient safety.
- Responsible for training and implementation of the patient safety, RCA, and HFEMA
- Maintains clear communications between the medical center and the NCPS
- Reviews, tracks, trends, and analyzes patient incident reports and scores each incident using the Severity Assessment Code (SAC)
- Performs root cause analysis
- Performs general patient safety programmatic functions
- Develops and coordinates internal review systems to assure that both clinical and administrative patient safety activities are in compliance with agency, accrediting standards, and regulatory requirements.
- Prepares and analyzes data, trends, issues of concerns, and makes recommendations for improvement.
- Maintains a strong working relationship with the VISN Patient Safety Officer
- Monitors and analyzes changes in mission goals, national patient safety programs guidance, new legislation, and technological advances to assess the impact of these changes on local works methods/procedures and to develop new systems which will address these changes.
- Initiates and revises administrative policies/procedures in the clinical care lines
The incumbent in this position will not be required to file a financial disclosure report.
Work Schedule: Monday through Friday, 8:00 am to 4:30 pm However, may be required to work unscheduled hours to meet the needs of the Veterans and the Medical Center.

- U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
- The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6)
- Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
- Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
- Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
- Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
- Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
- Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree in nursing or related field and approximately 2-3 year's of nursing practice/experience.
Preferred Experience: A RN with a MSN or BSN with 2 or moreyears of specialized experience in Patient Safety Management.Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.
Physical Requirements: Registered Nurse Functional Requirements: Heavy Lifting 50 lbs and over (intermittently); Carrying up to 45 lbs (intermittently); Repeated bending; Straight Pulling up to 45 lbs (intermittently); Straight Pushing up to 45 lbs (intermittently); Stooping (intermittently); Squatting (intermittently); Kneeling (occasionally); Use of both arms; Use of both hands; Use of fingers; Overhead work, both arms (intermittently); Prolonged walking (up to 4 hours/day); Prolonged standing (up to 4 hours/day); Sitting (up to 2 hours/day); Far Vision - correctable in one eye to 20/20 and 20/40 in the other; Near Vision - correctable to read newsprint at 13-16 inches; Ability to distinguish shades of colors; Hearing - ability to hear and understand the spoken voice from a distance of 8 feet. Ability to effectively utilize a stethoscope and distinguish breath and heart, and bowel sounds; Emotional and mental stability - ability to effectively perform the essential functions of the position and respond appropriately in emergency situations.
