The Director of Enterprise Care Management operationalizes the BayCare Health System strategies and plans for population health care management to assure care across the system is aligned with the goal of achieving the Triple Aim (Better Health, Better Care and Improved Cost).

The primary responsibility for this position is the development and oversight of the Continuous Care Management department and the education and IT oversight for the Episodic Care Management departments to assure consistent, efficient delivery of services throughout the healthcare continuum and demonstration of positive outcomes.

The Director will be accountable for overseeing that patient hand-offs, from site specific/episodic events to the ambulatory care setting, are coordinated and integrated among various locations through technology, data and processes. The Director will oversee the implementation of evidenced based, core and consistent care management practices across the system, which includes determining role functions and competencies; curriculum development, education and training; and innovation toward recognition as a national leader in care management.

To advance achievement of the Triple Aim, the Director will work and collaborate with various departments and levels to design and implement the care management program, operating model and practices and assure alignment of care management practices.

Required experience: 5 years recent experience in case, care or disease management (ambulatory)

Required Licensure: Master's degree in lieu of RN license is acceptable.
